JOB SUMMARY:
The Nurse Practitioner interacts with physicians and other referral sources within the community to facilitate resident referrals and provide primary care of such residents under the direction of the referring physician.
KEY RESPONSIBILITIES:
A. Practices medicine and nursing within the scope of license, training and credentialing and in accordance with the current standards of care.
B. Facilitates the referral of residents from physicians and other referral sources, including but not limited to, conducting admission assessments and discharge planning. Provides regular updates to Facility Administrator and clinical and marketing personnel regarding referral activity.
C. Provides primary care to residents under the direction of the referring physician, including diagnosis and treatment of acute, chronic, and long-term healthcare issues, in accordance with federal and state regulations.
D. When appropriate provides evaluation and treatment of residents that would otherwise require non-emergency hospital evaluation.
E. Develops professional working relationships with physicians and physician groups within the community in order to facilitate clinical consultation, collaboration and referrals.

F. Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations consistent with federal and state regulatory standards.
G. Educates patients and/or families about preventive care, medical issues and the use of prescribed medical treatments and/or medications.
H. Serves as a liaison between residents and healthcare providers.
I. Participates in quality assurance activities as requested by the Facility Administrator.
J. Interacts in a positive, productive manner with the Facility management team on operational, clinical and marketing issues.
K. Works collaboratively with the Facility management team and internal and external marketing staff regarding resident census, case mix (Medicare, Medicaid, Private Pay ratios), and marketing projects and issues on a weekly basis.
L. Represents the facility at community educational programs.
QUALIFICATIONS:
? Nurse Practitioner License.
? At least one (1) year of experience as a Nurse Practitioner.
? Excellent organizational and communications skills.
? Ability to communicate in English via phone, in writing and orally in conversation with staff, residents, family members and other external contacts.
